Attention: Information about the New Virtual Explanation of Benefits (EOBs)

Seven Corners is doing our part to protect the environment and we are now utilizing Virtual Explanation of Benefits (EOBs). To access your EOB, simply log into your MyPlan Account. If you have never been to MyPlan it is a website designed to allow you secure access to your health benefit plan and claim information. If you have not already set up a MyPlan Account, follow the instructions below to sign up:

1. Link here to http://myplan.sevencorners.com and you will go to a general log in screen
   
   
2. Click Setup New Account, or if you are an existing member, enter your user name and password
   
   
3. Enter your MyPlan ID (Your healthcare benefits program identification number found on your ID card) and PIN number or Date of Birth, if you have not been issued a PIN number.
   
   
4. After you enter this information you will be asked to choose a username and password for future use. You will only use your program identification number and birth date or assigned PIN the first time you enter.
